The reporting options below provide EMU community members an opportunity to share important information directly with appropriate offices. This is an internal (to EMU) reporting tool intended to address concerns in a safe, timely, and supportive manner. The goal is to connect individuals to the resources and people that they need. 

In all cases, if you believe there is an emergency or an immediate/ongoing threat to the safety of an individual or campus community please contact EMU Safety and Security at 540-432-4911 OR local emergency responders at 911.

- Campus Life Report: This form is mainly used by EMU Area Coordinators and Community Advisors to document incidents and concerns that occur in EMU residential areas. 

- CARE Team Report: Use this form to share a concern regarding a student's physical, mental, or social health and well-being. Any community member is invited to use this form to share concerns, including sharing concerns for one's own well-being.

- Conflict: Use this form if you would like to share a concern regarding a conflict that you’d like to talk to someone about to determine what the next best steps are. Submitting this conflict form is an informal request for conversation, not a report of a violation of university policy.

Bias/Discrimination: Use this form to share a report of an act of bias or discrimination that has occurred. *If the bias or discrimination has occurred on the basis of sex and/or gender, please use the Relationship Violence and/or Sexual Misconduct (Title IX) form. 

Safety and Security: use this form to report a concern or incident that impacts campus safety and security (theft, threats, violence, etc.)

Relationship Violence and/or Sexual Misconduct (Title IX): use this form to report a concern of possible sexual or gender-based harassment, discrimination, or violence. For more information on Title IX, please visit EMU's Title IX webpage
